Waivers: All team members and coaches must fill out and turn in waivers before they compete at any FCDA event. You can fill out the online waiver or download and print the waiver below. NOTE: If using printed waivers: Printed individual waivers must be turned in at coaches' check in at every competition you attend. If the "team waiver" is completed, you will not need the individual waivers. Also, the "team waiver" will be for the entire season so once filled out.
